Are you interested in managing drug therapy for a chronic health condition
under a collaborative pharmacy practice agreement with a physician?

Are you interested in more closely collaborating with a physician to enhance
patient outcomes and satisfaction while expanding your role as a pharmacist?

The Program offers an additional special content to help you engage in Collaborative Practices with Primary Care Clinicians:

  1. Presentations from pharmacists who are extensively engaged in Collaborative Practice arrangements.
  2. Physician discussing what information they want and need from pharmacists.
  3. Hands-on learning activities to guide you through the process.

The Program has three distinct components:

  1. Self-study modules and self-assessment (~12 hours), which must be completed no later than one day before the live training workshop.
  2. Live Virtual Seminar (~8 hours) conducted via Zoom on your selected day; attendance required.
  3. Post-workshop assessment and program evaluation.
