under a collaborative pharmacy practice agreement with a physician?

patient outcomes and satisfaction while expanding your role as a pharmacist?

The Program offers an additional special content to help you engage in Collaborative Practices with Primary Care Clinicians:

1. Presentations from pharmacists who are extensively engaged in Collaborative Practice arrangements.
2. Physician discussing what information they want and need from pharmacists.
3. Hands-on learning activities to guide you through the process.

The Program has three distinct components:

1. Self-study modules and self-assessment (~12 hours), which must be completed no later than one day before the live training workshop.
2. Live Virtual Seminar (~8 hours) conducted via Zoom on your selected day; attendance required.
3. Post-workshop assessment and program evaluation.

  Who should attend (target audience):

  Pharmacists, in a community, ambulatory care, health-system, or other practice setting who are interested in working in a collaborative relationship with a primary care physician. This Initial Collaborative Practice Course is specifically designed to meet the Florida Board of Pharmacy requirements for Collaborative Practice Certification as outlined in FS §465.1895.

  Accreditation Information

  The American Pharmacists Association is accredited by the Accreditation Council for Pharmacy Education as a provider of continuing pharmacy education.

  The twenty continuing education hours achieved in obtaining this certificate cannot be used twice with the Florida Board of Pharmacy. These hours will be applied solely to your certificate.

  To obtain full credit for the Initial Collaborative Practices Certification, registrants must complete the home study activities, complete and pass both the self-study and final assessments with a grade of 70% or better, attend the entire live workshop, complete the activity evaluations, and claim CPE credit (using the correct NABP eProfile Number, birthdate, and Florida pharmacist license number). Participants must claim credit once each component is completed. Once credit has been claimed, transcripts of the earned CPE credits will be available online on both the Learner’s CE Broker and CPE Monitor profiles as soon as these organizations post them. The Certificate of Achievement will be available online upon successful completion of all the necessary activity requirements on line.

  There will be two opportunities to complete each of the two assessments successfully. Please note that you will not be permitted to submit either assessment a third time. All assessment questions are randomized.

  Activity Goals and Learning Objectives

  The purpose of this certificate-training program is to prepare pharmacists with comprehensive knowledge, skills, and resources necessary to engage in Collaborative Practices. For this workshop portion, learners will be engaged in the following learning exercises: case study scenarios (building from simple to complex); active learning presentations; and interactive collaborative discussions.

  Chronic Disease Management in a Collaborative Practice

  The Pharmacist’s Patient Care Process model will be used to showcase the delivery of patient care across all practice settings. Learners will be guided through an interactive learning experience of collaborative chronic disease state management with their peers. This cooperative learning activity will begin with patient intake; followed by data assessment; next, developing a patient care plan; followed by implementing the designed plan; and finally, monitoring and documenting the patient’s progress and following-up with collaborators and the patient.

  Upon completion of these activities, the participant should be able to:

  • Collect sufficient subjective and objective data from the patient, patient’s records, and other collaborative healthcare professionals
  • Demonstrate an understanding of physical assessment procedures used to collect relevant patient data
  • Enter physical assessment data into the patient’s chart
  • Order lab tests based on a patient’s condition and need
  • Perform lab tests based on a patient’s condition and need
  • Enter lab test results data into the patient’s chart
  • Document your findings
  • Interpret relevant subjective and objective data to be able to assess the patient’s condition
    • Evaluate patient assessment findings
    • Evaluate lab value results
    • Evaluate information gathered from patients
    • Establish patient-centered goals
    • Create a care management plan for a patient in collaboration with the healthcare team
    • Safety, therapy effectiveness, and medication appropriateness
    • Adherence to plan and therapeutic recommendations
    • Document the outcome and evaluation of pharmacist-initiated patient interventions

    Collaborative Practice Agreements

    Another key part of this Initial Collaborative Practices Certificate Program is gaining some experience in creating a representative CPA that meets the requirements of FS§465. Learners will participate in collaborative presentations and learning exercises with the program faculty and peers of developing a CPA in which you will be performing different patient care functions in managing a covered chronic disease state or condition. Upon completion of these activities, the participant should be able to:

    • Detail the requirements for a pharmacist to become involve in a CPA as outlined by §465
    • Synopsize the conditions that pharmacists must meet to join in and practice under a CPA in Florida
    • Discuss potential barriers to implementing a CPA
    • Describe the key components of a CPA
    • Construct an example CPA
    • Discuss successful models of a collaborative practice
    • Identify the major components of a business plan
    • Prepare a model business plan outline for your collaborative practice
    • Identify your competitive and strategic advantages
    • Outline marketing strategies for your collaborative practice
    • Create an elevator speech or a "30-second commercial" for your service
