Generally, in almost all Civil Supplies Department at the Collectorate office or any other concerned office responsible for monitoring issue of rations, you will get the application form in a prescribed format, which you will have to fill up and submit to apply for a new ration card for your family.
In case, there isn't any such prescribed form for issue of new ration card in your locality then you can submit an application request letter with all the required details in English or the local language, as may be acceptable by the concerned office. Please enquire with them before going ahead with the preparation of the application and the documents copies required along with it.
Following format can be used for requesting a new ration card. It can be addressed to the Rationing Officer or the Civil Supplies Officer.
